Upload Your COVID-19 Vaccination Record
Please use this secure form to submit a copy of your COVID-19 vaccination card.
The COVID-19 vaccination is not mandated but strongly encouraged for all students, faculty and staff.
Why should I upload my vaccination record? If you are a close contact and the college has confirmed you are vaccinated you will not need to quarantine or stay home.
You must include an attachment(s) as well as additional contact information. Only upload once you have either received your second dose or the Johnson & Johnson one dose.
You will need your 900# to upload your vaccination record. You can log in and get it here: FIND MY 900#
